Mental Health Care Plans

If clinically appropriate, your GP can prepare a Mental Health Care Plan (MHCP). This is a structured document that outlines your mental health needs, treatment goals, and recommended course of care. A Mental Health Care Plan provides access to Medicare-subsidised sessions with a psychologist, mental health social worker, occupational therapist, or other eligible mental health professional.

Under Medicare, eligible patients may access up to 10 individual and up to 10 group therapy sessions per calendar year when referred under a Mental Health Care Plan. Your GP will discuss whether this is suitable for your circumstances and which type of mental health professional would best support your needs.

Please book a long consultation (30 minutes) when attending for mental health concerns.

This allows your GP adequate time to discuss your situation thoroughly, complete any relevant assessments, and prepare documentation without feeling rushed.

Ongoing Mental Health Support

Mental health care is rarely a one-off appointment. Our GPs provide continuity of care, meaning you can build a relationship with a trusted mental health doctor in Baldivis who understands your history and can support you over time. Regular follow-up appointments allow your GP to review your progress, adjust your care plan, and ensure you remain connected to the right services.

Where mental health concerns are linked to physical health conditions — such as chronic pain, cardiovascular disease, hormonal changes, or other long-term health issues — our team can coordinate your care across both areas. You can explore our Chronic Disease Management services for more information on how we manage complex, ongoing health needs.

Our practice also provides a welcoming and inclusive environment for all members of the Baldivis community. We are a safe and affirming space for individuals from diverse backgrounds, including members of the LGBTQI+ community.
