Appointments at Baldivis Family Medical Centre can be made online via HotDoc or by calling (08) 9591 8423 You can select your preferred GP and time when booking online. For telehealth consultations, please inform us when scheduling to ensure we allocate the appropriate time.

Whilst the practice will always make all reasonable efforts to alert patients of an important test result it is important for patients to know that this is sometimes not possible and that it is their responsibility to follow up their own test results. Do not assume all results are normal because “you haven’t heard anything”.
Concerns with patient safety, confidentiality and ethical reasons prevent receptionists from giving test results. Results can only be given out by a doctor. Please ensure to ask your Dr at the time of your appointment what follow up arrangements should be made to obtain your results.
If tests have been ordered, patients should double check that their correct contact details are on file at reception.
For patients to qualify for the higher-level Medicare rebates when they see a specialist, a GP must authorise the visit and write a referral. If you want the specialist Medicare rebate, please organise a consultation with your GP BEFORE you see the specialist. It is Medicare’s view that it is usually not appropriate for doctors to write referrals without first seeing the patient and it is illegal to back date referrals or medical certificates. Referrals expire after a set period and it is necessary to obtain a further GP review at that time in order to obtain a further referral extension.
